RPEV SHARE TECHNOLOGIES PVT.LTD insurance Plan Terms & Conditions: I HEREBY AGREE AS FOLLOWS:

By applying for RIDERS PILOT insurance Plan (Annexure 1), I read, understood and willing to obtain a RPEV SHARE TECHNOLOGIES PVT LTD insurance Plan on the terms and conditions as specified in this document, subject to my
eligibility criteria and fulfilling necessary documentation process with the Company. I accept the terms and conditions and request the Company to disburse the claim amount on the said terms. I hereby, declare that, the entries made by me in the Application Form are complete and true to the best of my knowledge and based on records. I, hereby, undertake to present the original documents immediately upon demand by the concerned authorities. I, hereby undertake to inform the Company, about any changes in information submitted by me, in the Application Form and any other documents, including change in addresses and phone nos., from time to time. I, hereby authorise, explicitly consent to Company to validate any information provided by me. I expressly agree and undertake and given my consent and hereby authorize the Company and/or any of its group companies, where I am recruited as on date (“RPEV ”) to deduct the insurance charge on a basis from my monthly payout, till the time I am recruited with the Company/ RPEV , or plan closure confirmation from me to the company RPEV , ,whichever is earlier, subject to applicable deductions and policies from time to time. I further agree that the Company at it’s discretion and based on the documentation provided may reject the claim amount and as per the Company’s discretion even in case of lack of few documents the Company may process Forty-five percent (45%) of the claim amount not exceeding INR 5,000 (Indian Rupees five thousand only). I hereby agree to apply for RPEV insurance Plan and provide my express consent to the terms and conditions. I hereby declare that I am in good health and do not suffer from any Pre-Existing medical conditions or critical illness covered under the policy.

RPEV SHARE TECHNOLOGIES PVT LTD insurance Plan

Policy Overview
The RPEV SHARE TECHNOLOGIES PVT LTD insurance Plan provides coverage for below mentioned scenarios:

Accidental Death+ Permanent Partial Disability+ Permanent Total Disability: INR 5,00,000 Temporary Total Disability/ Weekly benefit: Sum insured against Temporary total disablement is 24
times of the monthly salary/income or as mentioned in schedule whichever is less. Indemnity against

this coverage will be 1% of Total Temporary Disability sum insured or Rs. 5000 per week whichever is less for maximum temporary total disablement period of 80 weeks.
Transportation of Mortal remains: INR 5000 Ambulance coverage: INR 2000
Accidental Hospitalization and OPD: upto INR 40,000
RPEV SHARE TECHNOLOGIES PVT LTD insurance Charge: INR 5/-(inclusive of GST) per Rider per day on pro rata basis, for the number of days the vehicle is allotted to the Rider.
RPEV SHARE TECHNOLOGIES PVT LTD insurance Plan is applicable on the Riders who are active with Rider’s pilot and Vehicle is allotted to the Rider at the moment.
In any accident situation whatsoever, if any claim to be filed shall be filed within 7 days of the incident. No claim shall be accepted after the stipulated time period as mentioned.
If the wallet balance of the Pilot is negative in any case whatsoever. No payment will be made to the specific pilot under any circumstance.
